Thank you for being a member of Civitas Networks for Health®! We are requesting you and your team's feedback on your 2026 membership experience so far to help us to continuously improve, deliver high-value member benefits, and inform our programming for the remainder of the year.

We’ve designed this survey to be completed in less than 5-7 minutes, and more than one member of your team may respond, based on their personal experience and feedback.
