2025 Civitas Networks for Health® Member Satisfaction Survey

Thank you for being a member of Civitas Networks for Health®! We are requesting you and your team's feedback on your 2025 membership experience so far to help us to continuously improve, deliver high-value member benefits, and inform our programming for the remainder of the year.

We’ve designed this survey to be completed in less than 5-7 minutes, and more than one member of your team may respond, based on their personal experience and feedback.

Respondents opting to share their contact information will be entered into drawings for one of three prizes as a thank you. Civitas will be offering two $100 Amazon gift cards and a top prize of one free registration for the 2026 Civitas Annual Conference (valued at over $1000). Please note that only one prize will be awarded per organization.
