As a member of Civitas Networks for Health® (Civitas) we invite you to participate in part one of a two-part survey to help better understand Civitas member involvement in the Rural Health Transformation Program (RHTP). This short questionnaire will be distributed to all Civitas members and asks how your organization is positioned to support the RHTP, what offerings your organization can provide to the state(s) you serve, any key concerns that might affect participation, and what states will need to maintain progress once federal funding ends.
Part one is for our initial understanding of how Civitas members are engaging with states to support the success of the RHTP. Once the program is further along and states are underway with implementation, Civitas will issue part two of the survey, which will be more specific and tailored to the way the program is evolving.
Civitas members—including APCDs, HIEs, QIOs, HDUs, and RHICs—bring decades of rural health expertise and have built the data infrastructure and community relationships the RHTP needs for continued success beyond funding timelines. Your participation in this survey will help ensure that Civitas member expertise is central to RHTP planning and sustainability.
Please submit only one response per organization; we encourage internal discussion and consideration of multiple perspectives, but ask that each organization submit a single consolidated response.
We ask that responses be submitted by June 22, 2026. Your responses will be collected confidentially and reported only in aggregate, unless attribution is explicitly permitted in the questions below.
